Initial Consultation
A complimentary one-hour meeting with the older adult's family or responsible
party to discuss their concerns, the older adult's condition, and to preview
our services.
In-Home Assessment
An interview with the older adult with family members or responsible
parties present to assess the situation. During this meeting a gerontologist
will interview the older adult, use several psychosocial and environmental
measurement tools to determine their physical, mental condition, and will
tour and assess their home environment. All inquiries and tests are performed
with emphasis on the older adult's pride and dignity. Input from or consultation
with the older adult's primary care physician is highly recommended.

Assessment and
Care Plan Report
After the in-home interview, a team of gerontologists will analyze
the outcomes of the measurement tools and the information provided. A
confidential report that includes all results with recommendations for
a plan of care is provided.
Implementation Meeting
The assessment and care plan report, along with the primary physician's
input, is reviewed, and discussed with the family or responsible party.
A plan for caregiving support, home modification, nutrition enhancement,
medication management, medical supervision, and, if recommended, a change
in living arrangement is provided. Important terminology and service options
are also provided. The family or responsible party may choose to implement
the services themselves or Elder Support Services
will implement and organize the services for them.
Ongoing Monitoring
Elder Support Services provides ongoing
monitoring with personal visits and telephone calls to the older adult
to ensure that the support systems are providing the level of care that
meets their needs. The frequency of personal visits and telephone calls
are scheduled based on the level of service requested. Service providers
are automatically contacted in the event changes in services are required.
The family or responsible party receives a monthly summary showing visits,
telephone calls, and all actions taken.
Reassessment
A reassessment is performed when there is a change in the older adult's
status. A meeting is held with the family or responsible party to determine
if modifications are needed in care arrangements.
